• Detailed planning visit: Deeper discussion of expectations, long-term bone healing, and the lengthening and consolidation timeline. Review of medications, health history, and any necessary lab work or medical clearance from your local physician. Clear explanation of the surgical technique, hospitalization (typically 5-7 days), physiotherapy requirements (3-5 sessions weekly), scarring, potential complications (infection, delayed union, nerve irritation, joint contractures), and the expected total stay in Germany (3-6 months).
• Scheduling and logistics: Surgery date is confirmed. You receive written pre-operative instructions about stopping blood-thinning medications (aspirin, ibuprofen), smoking and alcohol restrictions, and fasting before general or spinal anesthesia. You arrange flights to Munich International Airport (MUC) or Frankfurt am Main Airport (FRA), book long-term accommodation near your chosen clinic (serviced apartments or medical tourist housing), and organize transport to and from the procedure.

In Germany

Before discharge, you are trained to use the PRECICE remote control (for internal nails) or to care for pin sites (for external fixators). You also receive a home exercise program to prevent joint stiffness and muscle weakness.

After Surgery (Recovery and Lengthening Timeline)

• Weeks 1-2: You are discharged from the hospital to your nearby accommodation. You begin active lengthening at a rate of 0.75 to 1.0 mm per day, divided into 3-4 sessions using the external remote control (for internal nails). Physical therapy sessions occur 3-5 times weekly, focusing on maintaining knee and ankle range of motion and preventing muscle contractures. You remain non-weight-bearing and use crutches for all mobility.
• Weeks 3-6: Lengthening continues daily. You have now achieved approximately 2-4 cm of height increase. You attend clinic follow-ups with X-rays every 1-2 weeks to monitor bone regenerate formation. Physical therapy adds resistance bands and light strengthening exercises. Some centers offer hydrotherapy to reduce muscle tightness. You may begin partial weight-bearing (25% of body weight) if X-rays show good regenerate quality.
• Months 2-3: Lengthening typically completes by the end of month 3, with a total height increase of 6-8 cm achieved. The consolidation phase begins, during which the new bone (regenerate) hardens and matures. Physical therapy shifts to strengthening, balance training, and gait re-education. You progress from non-weight-bearing to partial weight-bearing (50-75%) as X-rays show corticalization of the regenerate.
• Months 4-6: No further lengthening occurs. You continue consolidation, gradually transitioning to full weight-bearing without crutches or with a single cane. Physical therapy reduces to 2-3 sessions weekly, focusing on endurance and return to daily activities. By month 6, most patients can walk independently and return to low-impact activities such as swimming or stationary cycling.
• Months 6-12+: Full consolidation is typically achieved by 9-12 months. You can return to all normal activities, including sports and high-impact exercise. A final set of X-rays confirms complete bone healing. If desired, you may undergo optional nail removal surgery (an outpatient procedure under anesthesia with a 1-2 week recovery). Otherwise, the PRECICE nail can remain in place permanently with no long-term issues. The full maturation of the lengthened bone and surrounding soft tissues continues for 12-18 months after surgery.
